1095C Tax Form

When was the form sent?

A 1095c Tax Form was sent to employees no later than January 31, 2017. The form was emailed to employees that selected electronic delivery and mailed to employees that did not select electronic delivery.

What if I need another form?

You can obtain another 1095c form by logging onto healthefxforms.us/cw. Registration will be required the first time you log on.

W-2 

When was the form sent?

A W-2 was mailed to employees no later than January 31, 2017 to employees that did not elect the Paperless W-2 option. Employees that did elect the paperless option were given the ability to access their W-2 on Employee Self-Service mid-January 2017. 

What if I need another form?

If you did another form you can obtain one on Employee Self-Service.
